When you should use this form
You should report a complaint about cleanliness or food handling when the food business is:
- unclean or unhygienic
- handling, storing, or transporting food in an incorrect or unhygienic manner, which may result in food being unsafe.
Examples are:
- Incorrect storage condition for the food – for example, food that needs to be chilled kept at room temperature for more than 2 hours.
- Undercooked food.
- Cross-contamination between raw ingredients and cooked food.
- Poor personal hygiene of food handlers.

Note, the use of gloves is not a mandatory requirement under the current laws. Handwashing is the best way for food handlers to get rid of illness-causing bacteria and viruses.

If your complaint relates to the dining area or the toilet facilities at a food business being untidy or disorganised, inform the food business of your concerns first. The food business is also responsible for managing complaints lodged with them.
