To complete this form, you must be registered as a food importer with the Ministry for Primary Industries (MPI).

Before completing this form, you will need a copy of the GFSI-recognised certificate and a manufacturers declaration for each overseas manufacturer you need to be checked.

Note that some ready-to-eat frozen berry imports do not need this check. To find out what berries need this check, refer to section 7.1(1)B of the Food Notice.
You also do not need to do this check if you are importing from Australia.
The initial cost for the check is $155.25 including GST. Any additional time required (over one hour) will be billed at the rate of $155.25 (including. GST). You can pay by credit or debit card or by direct credit. A tax receipt is available on request.
