About this form
This form can be used to obtain approval for a new facility or to make changes to an existing approval.

Supporting documents for new facilities and change of ownership
If you are making a new application or changing facility ownership, you'll need to send us these documents with your application:
- A completed 'Trade Single Window – client registration application/update' form (TSW NZCS 224)
- If you don't already have a credit account with MPI, a credit account application [PDF, 300 KB]
- A copy of your company's Certificate of Incorporation available from the NZ Companies Register
- A letter of authorisation, if your company is to be the operator.
- Copies of current identity documents for all listed directors.
The documents you upload must have a combined file size less than 20 MB and any images must be clear. Most file types are accepted.
The documents are not required for other types of changes.
